500 Error After Posting (Feedback Request)

Hey Folks, 

I'm trying to gather some feedback from the forum. I've noticed that every so often (personally and in the logs) that after trying to create a new topic a 500 "not found" forum page shows. 

If you experience this while posting on the forum, please kindly post here so we can try to resolve the issue
